W-9 FORM (IRS)

The W-9 form, also known as a "Request for Taxpayer Identification Number and Certification Form", is an Internal Revenue Service (IRS) tax form.  The City requires a W-9 form to be completed, signed and on file for all independent contractors earning compensation from the City of Woonsocket.  

The W-9 form is used to confirm a person's name, address, and TIN (Taxpayer Identification Number).  For most, the TIN will be your Social Security Number (SSN).  It is important that the W-9 the City has on file contains current information.  In addition to using the information to generate 1099 tax forms, the City relies on the W-9 information provided to process and mail compensation payments to contractors.
